Sample Documentation
The following procedures were used during sampling and analysis to provide chain-of-
custody control during sample handling from collection through storage. Sample
documentation included the use of the following:
o Water sample field data sheets to document Chain-of-custody record sheets for
sampling activities in the field documenting possession and transfer of
• Labels to identify individual samples samples
Laboratory analysis request sheets for
documenting analyses to be performed
Field Logbook
In the Feld, the sampler recorded the following information on the water sample field data
sheet (see Figure A-2) for each sample collected:
O Project number e Calculated and actual purge volumes
G Client's name O Purging equipment used
a Location G Sampling equipment used
a Name of sampler • Appearance of each sample (e.g., color,
• Date and time turbidity, sediment)
* Results of field analyses (temperature, pH,
• Well accessibility and integrity specific conductance)
• Pertinent well data(e.g., casing diameter, • General comments
depth to water; well depth)
The water sample field data sheet was signed by the sampler and reviewed by the sampling
coordinator.
i
SAC,A:12185QM2.DOC-Mdsantos:[ A-5 Rev.d,917/98
