FACTSHEET <br /> FLAG CITY CHEVRON <br /> 6421 Capitol Avenue,Lodi, California <br /> 1.0. BACKGROUND INFORMATION <br /> The Flag City Chevron facility(the site) is located at the northeast intersection of Capitol Avenue <br /> and Thornton Road,approximately 500 feet south of California State Highway 12 and approximately <br /> 5 miles west of Lodi, California. The San Joaquin County Tax Assessor's Office lists the site's <br /> boundaries as Parcels 23 and 24 on Map 32 in Book 55(APN 55-320-23/24).The Flag City Chevron <br /> property is currently owned by the Bokides Family Limited Trust (Mr. Richard Bokides). The <br /> property has been utilized as a retail fueling station from approximately 1984; prior to 1984, the <br /> property was utilized as a saddle shop. The Flag City Chevron property currently consists of one <br /> building structure (mini-mart), six fuel dispensers, three 12,000-gallon gasoline USTs and one <br /> 12,000-gallon diesel UST. <br /> Three ground water monitoring wells(MW-1,MW-2 and MW-3)were previously installed at Flag <br /> City Chevron in May 1991 near the former UST and fuel dispensing areas as part of a preliminary <br /> environmental site assessment. <br /> 2.0. LEAKING UNDERGROUND STORAGE TANK INVESTIGATION <br /> In March and April 1995,four 12,000-gallon gasoline USTs,one 12,000-gallon diesel UST and the <br /> associated fuel dispensers and product piping were removed from the site.During the UST removal <br /> process, soil samples and grab water samples were collected from the excavation area. Gasoline- <br /> impacted soil was quantified beneath the southeast fuel dispenser area. Furthermore, floating <br /> petroleum sheen was observed in the UST cavity and gasoline-impacted and diesel-impacted ground <br /> water was quantified from grab water samples collected in the UST excavation area. <br /> Based on UST removal observations and analytical results,a subsurface investigation was required <br /> by the San Joaquin County Environmental Health Department(SJCEHD)by letter dated 25 January <br /> 1996. In order to expedite the investigation and cleanup of MTBE-impacted ground water, the <br /> California Regional Water Quality Control Board(RWQCB)issued Cleanup and Abatement Order <br /> No. 5-00-700, dated 01 March 2000. The following is a brief summary of all site assessment <br /> investigations that transpired at the site between July 1996 and November 2005: <br /> • Seventeen(17)groundwater monitoring wells(MW-4 throughMW-20)andten(10)ground <br /> water extraction wells (EW-1 through EW-10) were installed on and off-site to maximum <br /> depths of 150 feet bsg to monitor dissolved petroleum hydrocarbon concentrations in ground <br /> water, and to assess ground water flow direction and gradient. <br /> • Thirty-four(34)quarterly groundwater monitoring events were performed on wells MW-1 <br />
