FACTSHEET FLAG CITY CHEVRON 6421 Capitol Avenue,Lodi, California 1.0. BACKGROUND INFORMATION The Flag City Chevron facility(the site) is located at the northeast intersection of Capitol Avenue and Thornton Road,approximately 500 feet south of California State Highway 12 and approximately 5 miles west of Lodi, California. The San Joaquin County Tax Assessor's Office lists the site's boundaries as Parcels 23 and 24 on Map 32 in Book 55(APN 55-320-23/24).The Flag City Chevron property is currently owned by the Bokides Family Limited Trust (Mr. Richard Bokides). The property has been utilized as a retail fueling station from approximately 1984; prior to 1984, the property was utilized as a saddle shop. The Flag City Chevron property currently consists of one building structure (mini-mart), six fuel dispensers, three 12,000-gallon gasoline USTs and one 12,000-gallon diesel UST. Three ground water monitoring wells(MW-1,MW-2 and MW-3)were previously installed at Flag City Chevron in May 1991 near the former UST and fuel dispensing areas as part of a preliminary environmental site assessment. 2.0. LEAKING UNDERGROUND STORAGE TANK INVESTIGATION In March and April 1995,four 12,000-gallon gasoline USTs,one 12,000-gallon diesel UST and the associated fuel dispensers and product piping were removed from the site.During the UST removal process, soil samples and grab water samples were collected from the excavation area. Gasoline- impacted soil was quantified beneath the southeast fuel dispenser area. Furthermore, floating petroleum sheen was observed in the UST cavity and gasoline-impacted and diesel-impacted ground water was quantified from grab water samples collected in the UST excavation area. Based on UST removal observations and analytical results,a subsurface investigation was required by the San Joaquin County Environmental Health Department(SJCEHD)by letter dated 25 January 1996. In order to expedite the investigation and cleanup of MTBE-impacted ground water, the California Regional Water Quality Control Board(RWQCB)issued Cleanup and Abatement Order No. 5-00-700, dated 01 March 2000. The following is a brief summary of all site assessment investigations that transpired at the site between July 1996 and November 2005: • Seventeen(17)groundwater monitoring wells(MW-4 throughMW-20)andten(10)ground water extraction wells (EW-1 through EW-10) were installed on and off-site to maximum depths of 150 feet bsg to monitor dissolved petroleum hydrocarbon concentrations in ground water, and to assess ground water flow direction and gradient. • Thirty-four(34)quarterly groundwater monitoring events were performed on wells MW-1
