By 15 June 2001,please submit the revised Tables 1 and 2 with all the required <br /> information, as an addendum to the QMR. <br /> The QMR did not include a table of contents, or a list of tables, figures, and appendices. These items were <br /> identified as deficiencies in the 17 August 2000 Quarterly Monitoring Report Review letter of the Second <br /> Quarter 2000 Quarterly Monitoring Report, and you were requested to add these elements in future quarterly <br /> monitoring reports. Therefore, in order to provide an expeditious review of the groundwater monitoring data, the <br /> following items must be included in future quarterly monitoring reports: <br /> • Provide a Table of Contents for the QMR,including a list of tables and figures, and provide tabs and <br /> cover pages for the appendices. <br /> • Provide a groundwater flow diagram to document historic flow directions and magnitude. <br /> • Plot hydrographs for monitoring wells on the same vertical scale. <br /> • Provide a table listing all monitoring well details including: well number, date installed, casing <br /> diameter, casing material, slot size, surveyed elevation, top and bottom of screen, top and bottom of <br /> filter pack, depth to water, groundwater elevation, and aquifer zone. <br /> For the second quarter 2001 monitoring report,you must also develop a graphical and narrative site conceptual <br /> model (SCM)to explain the extent of groundwater contamination both laterally and vertically across the site. A <br /> description of site contamination from UST and dispenser areas to deep aquifer units was presented in Section <br /> 5.0 of the QMR. This work can be developed into the narrative SCM,and an east-west cross-section from <br /> MW-10 through the Chevron site and Rocky's domestic well (RDW-1) to MW-13 can be developed into the <br /> graphical SCM. <br /> The remediation system is operating to reduce on-site groundwater contamination, and decreasing trends are <br /> documented in the historical groundwater monitoring tables. The remediation system does not appear to be <br /> affecting off-site groundwater contamination, as observed at MW-11A and -1113, located next to RDW-1. As <br /> presented in multiple letters from this office, our goal is to clean up existing groundwater contamination, and <br /> prevent further contamination of high quality groundwater in deeper aquifer units. This can only be achieved <br /> after RDW-1 is properly destroyed. Therefore,you must continue efforts to provide a permanent water source <br /> for Rockies Restaurant so RDW-1 can be destroyed in a timely manner. <br /> Please notify Mr. Mike Infurna at the San Joaquin County Environmental Health Division (209)468-3454 and <br /> me at least two business days prior to any site work or sampling.
