By 15 June 2001,please submit the revised Tables 1 and 2 with all the required information, as an addendum to the QMR. The QMR did not include a table of contents, or a list of tables, figures, and appendices. These items were identified as deficiencies in the 17 August 2000 Quarterly Monitoring Report Review letter of the Second Quarter 2000 Quarterly Monitoring Report, and you were requested to add these elements in future quarterly monitoring reports. Therefore, in order to provide an expeditious review of the groundwater monitoring data, the following items must be included in future quarterly monitoring reports: • Provide a Table of Contents for the QMR,including a list of tables and figures, and provide tabs and cover pages for the appendices. • Provide a groundwater flow diagram to document historic flow directions and magnitude. • Plot hydrographs for monitoring wells on the same vertical scale. • Provide a table listing all monitoring well details including: well number, date installed, casing diameter, casing material, slot size, surveyed elevation, top and bottom of screen, top and bottom of filter pack, depth to water, groundwater elevation, and aquifer zone. For the second quarter 2001 monitoring report,you must also develop a graphical and narrative site conceptual model (SCM)to explain the extent of groundwater contamination both laterally and vertically across the site. A description of site contamination from UST and dispenser areas to deep aquifer units was presented in Section 5.0 of the QMR. This work can be developed into the narrative SCM,and an east-west cross-section from MW-10 through the Chevron site and Rocky's domestic well (RDW-1) to MW-13 can be developed into the graphical SCM. The remediation system is operating to reduce on-site groundwater contamination, and decreasing trends are documented in the historical groundwater monitoring tables. The remediation system does not appear to be affecting off-site groundwater contamination, as observed at MW-11A and -1113, located next to RDW-1. As presented in multiple letters from this office, our goal is to clean up existing groundwater contamination, and prevent further contamination of high quality groundwater in deeper aquifer units. This can only be achieved after RDW-1 is properly destroyed. Therefore,you must continue efforts to provide a permanent water source for Rockies Restaurant so RDW-1 can be destroyed in a timely manner.
