6421 Capitol Avenue `— - 2 - • 17 August 2000 <br /> In order to provide a thorough analysis of the groundwater monitoring data, the following clarifications <br /> must be included in future quarterly monitoring reports: <br /> • Provide a groundwater flow diagram to document historic flow directions and magnitude. <br /> • Provide hydrographs for monitoring wells MW-2,MW-6A, and MW-6B. <br /> • Provide a table to list all monitoring well details including: well number, date installed, casing <br /> diameter, casing material, slot size, surveyed elevation, top and bottom of screen, top and <br /> bottom of filter pack, depth to water, groundwater elevation, and aquifer zone. <br /> • Provide a table that presents all groundwater sampling data completed during the previous <br /> quarter. <br /> • Differentiate between Flag City monitoring wells, and Three B's monitoring wells. <br /> Groundwater monitoring logs and Chain-of-Custody forms should follow these same <br /> determinations. <br /> • Change the "Baker Tank" label on Table 7 to `BX-1" and include on Table 3 with other <br /> groundwater analytical results. <br /> • Provide a table to document the date and volume of wastewater hauled off-site for treatment. <br /> • Provide a Table of Contents for the QMR, including a list of Tables and Figures. <br /> As stated in our previous correspondence, you must continue efforts to provide a permanent water source <br /> for Rocky's Restaurant, so Rocky's Restaurant well can be properly destroyed. Previous correspondence <br /> by AGE estimated that Rocky's Restaurant would be connected to the Flag City water system by <br /> 15 September 2000. Mr. Scott Rishwain, the attorney for the Bokides Family Limited Partnership, <br /> included a letter update in the QMR stating that the proposed annexation of the Rocky's Restaurant site <br /> should be approved in July 2000. However, the letter update did not state if the 15 September 2000 date <br /> would be met. By 31 August 2000, and then by the 15'h day of every month until a permanent water <br /> source is supplied to Rocky's Restaurant, submit an update on the work completed to provide a <br /> permanent water source for Rocky's Restaurant. <br /> Please notify Mr. Mike Infurna at the San Joaquin County Environmental Health Division and me at <br /> least two business days prior to any site work or sampling.
