Capitol /Paddock PI. <br /> Lodi, CA., 95242 <br /> On February 24, 1998 San Joaquin County Public Health Services, <br /> Environmental Health Division (PHS-EHD) issued an approval to the "Work Plan <br /> Addendum - January 1998" as submitted by your consultant Advanced <br /> GeoEnvironmental (AGE) on February 4, 1998. In a telephone conversation with <br /> Tim Cuellar of AGE on June 1, 1998, PHS-EHD advised that the work plan was <br /> to be enacted and the placement of the approved monitoring wells was to be <br /> started by June 15, 1998. This requirement has not been complied with. <br /> PHS-EHD hereby directs you to enact the work plan approved on February <br /> 24, 1998 by installing all of the proposed monitoring wells by June 26, <br /> 1998. The wells are to be developed and sampled by June 30, 1998 and the <br /> report of findings and the Second Quarter Monitoring Report (April, May, <br /> June - 1998) are to be submitted to PHS-EHD by July 20, 1998. <br /> In order to become and/or remain eligible to receive reimbursement for <br /> investigation and remedial costs associated with the unauthorized release from <br /> the underground storage tanks you previously owned and operated, you must be <br /> in compliance with directives issued by PHS-EHD. <br /> A boring permit is required for each parcel monitoring wells are to be installed <br /> on. Wells proposed to be placed in San Joaquin County Right of Way require an <br /> encroachment permit issued by the Department of Public Works in addition to <br /> the PHS-EHD boring permit. <br /> Please submit all of the required forms and a $89 fee with each PHS-EHD boring <br /> permit application in enough time to allow for the processing of the forms and the <br /> completion of the required drilling by June 26, 1998. <br /> A Division of San Joaquin Counry Healrh Care Services <br />
