MONITORING AND REPORTING PROGRAM -3 - . FLAG CITY CHEVRON, 6421 CAPITOL AVENUE,LODI SAN JOAQUIN COUNTY REPORTING As required by the California BusinessProfessions athe r subordinate and signed by the de Sections 6735, 7835, and 7835 , all reports shall be prepared by a registered professional o registered professional Quarterly reports shall be submitted to the Regional Board by the last day of the month following the end of the calendar quarter(31 January, 30 April, 31 July, and 31 October) until such time as the Executive Officer determines that the reports are no longer necessary The Discharger shall notify the Board within 48 hours of any unscheduled shutdown of any remedial system(s) When reporting data, the Dischargerereadilyfd information Thetabular data shall be the date, the constituents, and the concentrations ar summarized in such a manner as to illustrate clearly compliance with this Order Each quarterly report is to include the following minimum information (a) A description of the groundwater sampling measured before,field logs At a uring, and after purging,Meld logs shall contain water quality par method of purging, depth of water, volume of water purged, etc , (b) Groundwater contour maps for all groundwater zones, if applicable, (c) Iso-contour contaminant concentration maps for all groundwater zones, if applicable, (d) A table(s) listmg all monitoring well details including well number, date installed, casing slot size, surveyed elevation, reference elevation, top and bottom diameter, casing material, of screen, top and bottom of filter pack, and aquifer zone The screen and filter pack intervals information may be presented graphically in a figure (e) A table or groundwater flow diagram showing historical flow directions and gradients, (f) Cumulative data tables containing the water quality analytical results, depth to groundwater, and groundwater elevations, (g) A copy of the laboratory analytical data report, (h) The status of the groundwater extraction operational data including the total volume of extracted and treated groundwater, cumulative information on the mass of contaminant ctiveness of removed from the subsurface, ertamin ao thelng oper tion and maintenance of the system,remediation and system, and any field notes p g (1) If applicable, the reasons for and duration of all interruptions 1n the operation of any remedial system(s), and actions planned or taken to correct and prevent interruptions
