31 August 2005 <br /> AGE-NC Project No 96-0232 <br /> Page 10 of 30 <br /> RWQCB by letter dated 06 November 2000 A copy of NPDES permit no CAG915001 is <br /> included with Appendix G <br /> • March 2002 - The Rocky's Restaurant domestic water well (RDW-1) was abandoned <br /> utilizing blast perforation and casing ripping techniques under SJCEHD permit Well <br /> abandonment activities were performed by Water Well Technology Inc under the direct <br /> supervision of the SJCEHD and documented by representatives of AGE and the RWQCB <br /> A report of findings (Quarterly Report - First Quarter 2002, dated 15 January 2001) <br /> documenting investigation activities performed in July 2000 was prepared by AGE and <br /> submitted to the SJCEHD and the RWQCB <br /> • October 2002 - Five additional, single completed ground water monitoring wells (MW-14 <br /> through MW-18) were Installed off-site at the Rocky's Restaurant (Figures 2 and 3), well <br /> MW-16 was single-completed at depths between 5 and 25 feet (First Water Zone), wells <br /> MW-14 and MW-17 were single-completed at depths between 40 and 60 feet(Second Water <br /> Zone), and wells MW-15 and MW-18 were single-completed at depths between 65 and 75 <br /> feet (Rocky's Upper Zone) Well construction details and boring logs for MW-14 through <br /> MW-18 are In Appendix F, no soil samples were collected during installation of MW-14 <br /> through MW-18,however,MTBE was detected from all five wells at concentrations as high <br /> as 220 µg/1 during the 4"quarter 2002 ground water monitoring event A report of findings <br /> (Quarterly Report-Fourth Quarter 2002,dated 31 January 2003)documenting investigation <br /> activities performed in October 2002 was prepared by AGE and submitted to the SJCEHD <br /> and the RWQCB <br /> Two additional soil borings(P36 and P37)were drilled on-site to depths of 50 feet bsg, one <br /> soil boring(P-38)was drilled off-site towards the north at McDonald's Restaurant to a depth <br /> of 50 feet bsg(Figure 2) Boring logs for P36, P37 and P38 are in Appendix F Soil samples <br /> were not collected from P36, P37 or P-38, however, laboratory analysis of grab water <br /> samples detected TPH-d from P36 at concentrations as high as 79 µg/l, TPH-d,toluene and <br /> xylenes from P37 and P38 at concentrations as high as 1,700 µg/1, 1 µg11 and 0 78 µg/l, <br /> respectively A report of findings (Sate Assessment Report-Fourth Quarter 2002, dated 12 <br /> March 2003) documenting soil and grab water sampling activities in October 2002 was <br /> prepared by AGE and submitted to the SJCEHD and the RWQCB <br /> • January 2004-Four additional soil borings(P39 through P42)were drilled on-site to depths <br /> of 50 feet bsg,one soil boring(P-43)was drilled off-site on Banner Avenue to a depth of 50 <br /> feet bsg(Figure 2) Boring logs for P39 through P-43 are in Appendix E TPH-d was detected <br /> in soil samples from P41 (30 feet) at a concentration of 1 7 mg/kg, MTBE was detected in <br /> soil samples from P40A (30 and 45 feet), P41 (30 feet) and P43 (30 feet) at concentrations <br /> as high as 0 39 mg/kg MTBE was detected in grab water samples collected from P39 at <br /> concentrations as high as 570 µg/l,from P40A as high as 120µg/l,from P41 as high as 1,900 <br /> A&anved GeoEnviroomentsl,Inc <br />
