The designated operator employee training for employees was not performed in a timely manner. The most recent training was conducted on 03/22/11. Last year's training was conducted in 01/22/10 - two months late. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and ensure training is performed every 12 months. 24, 25, & 26. Annual monitoring system certification, leak detector testing, and spill container testing were last performed on 03/29/10 and was completed today, one month past due. These tests are required once every 12 months - within the same calender month. Take all necessary precautions to ensure testing is performed in a timely manner. Submit to SJC EHD within 30 days (05/06/11): 1. one copy of the completed "Return to Compliance Certification" form 2. a written corrective action statement for the above violation Notes: - received Certification of Financial Responsibility and Letter from Chief Financial Officer for 2011 ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
