AUG 15201, Bravo CONY Retrofit Plan 8/21/12 <br /> Plan for control of the CONV Retrofit Plate <br /> ENVIRONMENTAL HEALIVH <br /> r,-_DnPT- APNT <br /> The Conversion Frame Retrofit Plate Kit is required only in very specific situations where existing <br /> electrical conduits are locked in concrete just outside the UDC opening. In this case, in order for the <br /> UDSCCS to be installed and function properly, holes must be cut or drilled in the conversion frame to <br /> accommodate the existing conduit lines and possibly the E-Y adapters. Bravo ensures absolute <br /> environmental protection by providing a fiberglass plate kit that is permanently bonded onto the <br /> conversion frame panels to cover the holes/cuts and to also seal directly to the conduit lines using <br /> fiberglass epoxy. The installation of this Retrofit Plate kit maintains the design and function of the <br /> drainage panels by containing any leaks from within the hydraulic cabinet —especially any fluids that <br /> may run down or along conduit lines- and channeling them into the interior of the UDC below. <br /> Bravo process for policing the CONV Retrofit Plate installation for existing external conduits. <br /> A) Bravo is contacted directly by the installer, or Bravo contacts the installer to communicate <br /> directly. By email, or by phone followed by email. Bravo does not communicate procedure or <br /> installation directions verbally. Bravo will maintain a paper trail history of each installation it <br /> authorizes. <br /> B) A completed Conversion Frame Evaluation Form will be sent to Bravo to help with evaluation <br /> and for records. <br /> C) For evaluation, the installer or an agent must send to Bravo complete, clear & unobstructed <br /> images of the conduit areas in relation to the existing UDC's. Bravo asks for closeup images <br /> and also images with the entire dispenser within the cameras frame. <br /> D) The installer is verified to be Bravo-Certified before authorization & installation. <br /> E) The installer must use only Bravo-supplied CONV Retrofit Plate components and materials for <br /> the Bravo Conversion Frame tailored to the specific installation on a per address/application <br /> basis after site evaluation. <br /> F) The installer may only install the CONV-132000 Conversion Frame along with the CONV <br /> Retrofit Plate kit when authorized in writing by Bravo letterhead on a per installer, per address <br /> and per application basis. In some cases the directions may need to be modified slightly to <br /> better apply to the specific layout of the conduit area. <br /> G) The installer must follow only our written instructions for this specific installation procedure. <br /> H) Post-installation the installer must send to Bravo images of the completed installation for <br /> review and further action as required. To be determined by Bravo engineering staff. Images <br /> are to match the pre-installation images as closely as possible to provide a before & after set. <br />
