Sl 0077430 (including: replacement of dispensers and UDCs 3-4 and 7-8; converting 87 -product siphon underground <br />storage tank (UST) to diesel UST; and installing piping to new dispensers. <br />Since underground utilities were not included on the site map, provide the USA Dig confirmation number prior <br />to excavation activities at this facility. Refer to page 1, #2, "Application for Underground Storage Tank Retrofit <br />or Piping Repair Permit". <br />2. Submit the "Backfill Excavation Certificate" as required by the San Joaquin County Building Department. Refer <br />to page 1, #6, "Application for Underground Storage Tank Retrofit or Piping Repair Permit". ( A copy of the form <br />has been attached.) <br />Page 3 -List all components to be installed to convert the current 87 -product siphon UST to a diesel UST. <br />4. Page 3 - Describe how the lines will be plugged to the master 87 -product UST. (Are there plans to uncover the <br />87 -product siphon/diesel UST? If so, then include removal of the pipeline and collection of soil samples beneath <br />the removed piping.) <br />5. Page 3 — List the Red Thread IIA piping to be installed <br />6. Page 3 — List the part numbers of the Smith FRG 3" over 2" product lines <br />7. Page 3 — Indicate that there are two Bravo B8000 UDCs <br />8. Page 3- Indicate that there are two Gilbarco Encore 700 3+1 dispensers <br />9. Page 3 — List the model numbers to be used for the product and electrical penetrations <br />10. Page 3 — List the make and model number of the line leak detector to be installed <br />11. Page 3 — List sensor make and model numbers if new sensors will be installed <br />12. Page 4— Number 7 —answer "Yes" or "No" <br />13. Page 4— Number 9 —Describe how the soil will be handled if found to be contaminated. It cannot be replaced <br />into the excavation, as described <br />14. Page 5 — In the blank column on the far left, beneath "Dispenser Piping and Stockpile Samples", indicate the <br />number of 'Dispenser Samples', number of'Piping Soil Samples', and number of 'Dispenser & Piping Stockpile <br />Samples' <br />15. Page 5 — In the column (titled Tank #) on the far left of "Table #2 Recommended Minimum Verification Analyses <br />for Underground Storage Tank Investigations" indicate the total number of samples to be analyzed for the <br />constituents listed Table 2. <br />16. Page 6 — Resubmit a legible copy of this page <br />17. Provide the Vaporless (VMI) certification of the service technician <br />18. Provide Veeder Root certification of the service technician <br />19. Product & Vapor Piping Plan (F-4) — Currently illustrates that the diesel product lines are going to the existing <br />dispensers 1-2 and 5-6 instead of existing dispensers 3-4 and 7-8.
