In addition, the sampling analytical results must be submitted within 30 days after the UDCs <br /> and piping sampling. Only results that come directly to the EHD from a third party California <br /> certified laboratory with affixed chain of custody will be accepted by EHD. <br /> 5. Within 30 days from each shipment of hazardous waste, all pertinent hazardous waste disposal <br /> documentation (rinsate and all residual product disposal) must be submitted to the EHD. <br /> 6. Secondary containment testing for STP sumps tank#1 and tank#2 sumps (where piping has <br /> been terminated), for the newly installed UDCs 3/4 and 7/8, and the installed diesel piping is <br /> required six months after installation. Secondary containment testing for tank#1 fill sump <br /> maybe required six months after installation if it's determined that such testing is needed during <br /> its installation. Provide 48 hours notification to the EHD when this test is scheduled. <br /> 7. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be issued <br /> to the installation company), or a Tank Tester's license issued to the individual by the State <br /> Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid, current California UST Service Technician <br /> certificate issued by the International Code Council (ICC), and <br /> c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being tested, repaired, or serviced. <br /> 8. Individuals installing underground storage tank system components must possess or work <br /> under the direct and personal supervision of an individual physically present at the work site <br /> who possesses all of the following: <br /> a. A valid, current contractor's license issued by the Contractors State License Board, and <br /> b. A valid, current UST Installation/Retrofitting certificate issued by the International Code <br /> Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) being <br /> installed. <br /> 9. Log into the California Environmental Reporting System (CERS), update and submit the "UST <br /> Monitoring Plan" for diesel tank for approval. <br /> 10. Log into the California Environmental Reporting System (CERS), complete and submit a "UST <br /> Certification of Installation/Modification" form for the underground storage tank and piping <br /> within 30 days. <br /> 11. All components must be certified by an independent testing organization and labeled as <br /> required by the organization (UL, ULC, ANSI, API, ASTM, etc.). See California Code of <br /> Regulations (CCR) Title 23, Chapter 16, Table B for a complete list. <br /> 12. Permit expires 180 days from the date of approval. If an extension to the expiration date is <br /> required, a written request must be submitted to this office prior to the expiration date. <br /> 3 of 4 <br />
