Center St. <br /> Stockton, CA. , 95206 <br /> On August 12, 1997 San Joaquin County Public Health Services , Environmental Health Division <br /> ( PHS-EHD) notified your consultant, Foothill Engineering, that your work plan for site investigation <br /> was approved and the boring application and $89 fee was to be submitted then. This requirement <br /> has not been complied with. <br /> This is your official notification that you are out of compliance with your required leaking <br /> underground tank investigation and, as of this date, you have ninety (90) day to come into <br /> compliance. Failure to comply may result in you becoming ineligible to receive <br /> reimbursement under the State Water Resources Control Board's Cleanup Fund and result <br /> in formal enforcement action (referral to the San Joaquin County District Attorney's <br /> Environmental Prosecution Unit) for failing to follow directives issued by PHS-EHD. <br /> The enclosed boring permit application should be completed , signed by a C-57 drilling contractor, <br /> and submitted to PHS- EHD with a $89 check before April 27, 1998 . <br /> PHS- EHD highly recommends that you re-evaluate your current non-compliance status and take <br /> swift and positive actions to return to your previous eligible and compliant position.
