(Condor) <br /> on your behalf In the above-referenced work plan, Condor discusses how grout <br /> inadvertently entered the well screen during construction of monitoring well MW-3, and <br /> proposes to destroy MW-3 by over-drilling the borehole to a depth of approximately 52 <br /> feet below ground surface, removing all well materials, and backfilling the borehole with <br /> cement/five-percent bentonite grout using a tremie pipe. Condor further proposes to <br /> replace MW-3 by installing MW-3R approximately ten feet south of the original location <br /> for MW-3 using methods described in Monitoring Well Installation Work Plan, dated <br /> August 11, 2004, and approved by EHD on August 25, 2004. <br /> EHD approves Condor's work plan for the destruction of MW-3 and re-installation of <br /> MW-3R approximately 10 feet south of the original borehole for MW-3. Please submit a <br /> well destruction permit application and fee of$60.00 to EHD, and notify EHD at least 72 <br /> hours prior to commencing any fieldwork. Permit Number 0039601 may be used for <br /> installing MW-3R if the same drilling company, V & W Drilling, is installing the well. <br /> However, EHD must have documentation that V & W Drilling has workers' <br /> compensation insurance beyond October 1, 2004, as indicated on the Licensed <br /> Contractors Declaration form. - # 1 <br /> EHD is extending the date for completing the fieldwork consisting of well destruction, <br /> well installation, development, surveying, and initial quarterly groundwater monitoring to <br /> December 20, 2004; and EHD is extending the date to submit the monitoring well <br /> destruction and monitoring well installation reports to January 20, 2005. <br />
