Contact the local Fire Department and San Joaquin Valley Air Pollution Control <br />District for any applicable permits. <br />2. Any changes shall be submitted to this office for approval prior to commencing work. <br />All new addenda supercede the previous ones. <br />3. At least one person who holds the International Code Council (ICC) UST Service <br />Technician certification and manufacturers' certificate of training for all components <br />installed or tested must be onsite when work is performed. <br />4. Each person installing or testing any component must hold a certificate of training <br />from the manufacturer of that component. <br />5. EHD must witness the functionality of the new sensor to ensure that monitoring <br />meets the requirements of California Code of Regulations Title 23 and the Health & <br />Safety Code. <br />6. Allow 48-hour notice for all appointments. <br />7. Complete and submit a copy of a UST Installation Certificate of Compliance (Former <br />Form C) for the 87/91 tank no later than 30 days after work is completed. <br />8.
