(Cambria) prepared this letter on behalf of Equiva <br /> Services LLC (Equiva) in response to the San Joaquin County Department of Health <br /> Services/Environmental Health Division (PHS/EHD) June 1, 2000 letter regarding the above <br /> referenced site. This letter requests a second work plan addendum. Provided below is a work <br /> plan addendum to Cambria's August 26, 1999 Site Investigation Work Plan. Elements of the <br /> original work plan and the first work plan addendum that are altered by this addendum are <br /> discussed below. Elements of the original work plan and the first addendum that are not <br /> discussed below will be performed as specified in the original work plan or the first work plan <br /> addendum. <br /> Work Plan Addendum <br /> The June 1, 2000 PHS/EHD letter requests we drill four soil borings instead of the two we <br /> originally proposed. We agree to drill four borings at the locations suggested by PHS/EHD. <br /> The proposed locations are shown on the attached site map (Figure 2). These boring <br /> locations are approximate and may be relocated slightly due to interference with existing site <br /> features. The boring locations may also be relocated slightly based on field discussions with a <br /> Oakland, CA PHS/EHD representative. <br /> San Ramon, CA <br /> Sonoma, CA <br /> Portland, OR We agree to perform laboratory chemical analyses requested in PHS/EHD's June 1, 2000 <br /> letter on soil and groundwater samples collected from the proposed four soil borings. <br /> Cambria <br /> Environmental <br /> Technology,inc. We plan to adhere to the schedule outlined in PHS/EHD's June 1, 2000 letter. However, if <br /> there are unforeseen delays we encounter as we proceed with our investigation we may request <br /> 270 Perkins Street that PHS/EHD grant us an extension to one or some of the due dates.
