Prom:STANTEC 9168610421 09/2412 10:51 #386 P.003I004 <br /> Stantet <br /> Corrective Action Plan <br /> Stockton Charter Way Commingled Plume No. CP0057 <br /> June 19, 2012 <br /> 4.0 SYSTEM INSTALLATION SCOPE OF WORK <br /> 4.1 WELL INSTALLATION AT CHEVRON FACILITY#92033 <br /> The proposed scope of work includes the installation of ten sparge wells (AS-3 through AS-12) <br /> and five vapor extraction wells (VE-2 through VE-6) prior remediation system installation. The <br /> new wells will be in addition to the existing two sparge wells (AS-1 and AS-2), one vapor <br /> extraction well (VE-1), and two piezometers (PZ-1 and PZ-2). The proposed well locations are <br /> presented in Figure 2 and will be finalized on-site upon approval based on field conditions. <br /> 4.1.1 Preliminary Field Activities <br /> Well installation will commence under the access agreement currently in place with the property <br /> owner. A schedule of field activities will be communicated to the property owner and tenants a <br /> minimum of fourteen days prior to field activities in order to minimize potential disruptions to <br /> normal station activities. <br /> 4.1.1.1 Permitting <br /> Prior to performing the well installation activities, all necessary permits will be obtained from the <br /> San Joaquin County Environmental Health Department. <br /> 4.1.1.2 Site Specific Health and Safety Plan <br /> As required by the Occupational Health and Safety Administration (OSHA) "Hazardous Waste <br /> Operations and Emergency Response" guidelines (29 CFR 1910.120) and by California <br /> Occupational Health and Safety Administration (Cal-OSHA) "Hazardous Waste Operations and <br /> Emergency Response"guidelines (CCR Title 8, Section 5192), the contractor will prepare a site- <br /> specific Health and Safety Plan (HASP). At a minimum, the HASP will define the proposed <br /> subsurface investigation activities, describe physical and chemical hazards that may be <br /> associated with the work, provide a map to the nearest emergency medical facility, and include <br /> material safety data sheets for any hazardous chemicals that will be used or produced during <br /> the work. A copy of the HASP will be available on-site at all times during field work. The field <br /> staff and subcontractors performing field activities will review the HASP prior to beginning field <br /> operations, as described in this CAP, at the Chevron Facility#92033- <br /> 4.1.1.3 Utility Survey <br /> All well locations will be marked or cleared of any underground facilities prior to drilling activities. <br /> As required by the California Government Code 4216, Underground Service Alert (USA) North <br /> and 811 will be notified at least two working days up to 14 calendar days before any drilling <br /> activities. Well locations will be marked with white paint to indicate the area of <br /> excavation/drilling. This will allow USA North members, which includes local utility companies, <br /> to identify and mark the horizontal path of their underground facilities located within the area of <br /> drilling.
