The SVES should be re-evaluated to address the deeper <br /> contamination at this site . A Work Plan to define the vertical extent of contamination <br /> and to further evaluate groundwater contamination and remediation at this site <br /> should be submitted to PHS/EHD by February 28. 2000. <br /> A site map showing the locations of the previously removed USTs, piping and dispensers, <br /> location of the new USTs, piping and dispensers, all borings, VWs, MWs, etc ., should be <br /> submitted with each QMR as previously requested (see PHS/EHD letter dated October <br /> 12, 1998) . Also, provide PHS/EHD with a quarterly report on the status of the SVES <br /> (inflow concentration measurements, outflow, run hours, etc.) . <br /> Continue quarterly monitoring at this site for TPHg, BTEX, and Oxygenates until <br /> otherwise directed by PHS/EHD.
