The work outlined in this <br /> Work Plan has not been done. The work must be completed within 90 days of the date of <br /> this letter. If the work is not completed within that time frame the above referenced site <br /> will not be in compliance with PHS/EHD directives and reimbursement for corrective <br /> action costs from the State Water Resources Control Board Cleanup Fund may be <br /> jeopardized. <br /> A well permit application and $89. 00 permit fee should be submitted to PHSIEHD as <br /> soon as possible. After the permit has been issued the work can be scheduled.
