VELMA MASTERSON AUG 2 S 2006 BENETO INC PO BOX 980220 WEST SACRAMENTO CA 95798 RE : DARPETRO, INC Site Code: 1060 749 E CHARTER WAY STOCKTON, CA 95202 San Joaquin County Environmental Health Department ( EHD ) has received and . reviewed Revised Workplan for Vertical Groundwater Investigation (RWP), dated 05 July 2006 , and Addendum to Revised Workplan for Vertical Groundwater Investigation (AWP) dated 03 August 2006 , both prepared and submitted by Lee & Pierce, Inc. (L&P) on your behalf. The RWP is approved as amended by the AWP. Submit a well permit application with the $89 . 00 application fee and $285 . 00 grout inspection fee ($374 . 00 total) to this office by 25 September 2006 , along with a date scheduled for installing the approved monitoring wells . The wells should be installed within 75 days of date of this letter. By letter dated 22 May 2006 , EHD had approved the 18 April 2006 work plan for testing the soil vapor extraction (SVE) system on your site . To date, the approved scope of work has not been scheduled . Submit a schedule no later than 18 September 2006 for conducting the SVE test and have the test performed no later than 75 days following the date of this letter. The EHD has directed by letters dated 08 February 2006 and 22 May 2006 submittal of a report on the operation of the SVE system formerly in use on your site by 01 March 2006 , and 05 July 2006 , respectively. Your new consultant, Mr. John Lane of L&P verbally requested an extension of the due date. Submit the report no later than 18 September 2006 . Effective January 2005, electronic versions of all reports (quarterly, results of investigations , work plans, etc. ) must be submitted to the State Water Resources Control Board database program known as GeoTracker. To date , the first and second quarter reports for 2005 and approved work plans have not been submitted . Submission of reports to GeoTracker is required under California Code of Regulations , Title 23, Division 3 , Chapter 30, Sections 3890 — 3895 . You are directed to submit the omitted reports to GeoTracker by 18 September 2006 .
