Charter Way <br />Stockton, CA <br />The San Joaquin County Environmental Health Department (EHD) has requested and received <br />concurrence with a "no further action required" (closure) determination from the Central Valley <br />Regional Water Quality Control Board. This is to notify you that Section 25297.15 (a) in Chapter <br />6.7 of the Health and Safety Code includes a requirement that EHD (local agency) not issue a <br />closure letter, or make a determination that no further action is required with respect to <br />"hazardous substance" underground storage tank release sites unless all current record owners <br />of fee title to the site have been notified of the proposed action by the primary or active <br />responsible party. <br />Please provide EHD certification in writing that all record fee title owners have been notified of <br />the request for closure. If you are the sole property owner, provide a statement of that fact to <br />EHD, to fulfill this requirement (form enclosed). <br />In addition, prior to a closure/no further action required letter being provided to you, the <br />monitoring wells installed during the site investigation must be destroyed under EHD permit and <br />inspection. After reviewing the data on these wells, MW -1, MW -6,, MW -7,- MW -106, MW -107, <br />MW -108, MW -109, MW -208 and MW -209 must have the well contents removed, typically <br />through drilling them out, and grouted. Wells MW -2, MW -4, MW -5, MW -11, MW -104, MW -110, <br />MW -111, MW -202 and MW -210 can be pressure grouted with the required below surface cut off <br />and mushroom cap as stated in the San Joaquin County Well Standards. Any deviations from <br />the well standards must be justified. Please have your consultant or a C-57 licensed driller, <br />submit two EHD permit applications and $405 permit and inspection fees per permit (total of <br />$810).
