Sacramento St. Lodi, CA 95240 Program: HW <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> On site to conduct a routine hazardous waste(HW) inspection. <br /> #8 This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store, dispose <br /> of, transport or offer for transportation, hazardous waste without an EPA ID number. Immediately <br /> contact DTSC and reactivate your EPA ID number and submit evidence to the EHD by 11-22-10. A form <br /> has been provided. Minor-Notice to Comply <br /> #20 Three five gallon buckets of used oil were not labeled. All hazardous waste containers shall be <br /> marked with the following information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties <br /> 4. physical state <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required <br /> information. Since it cannot be determined how long the three five gallon buckets of used oil have been <br /> on site, immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest <br /> and submit a copy of the manifest to the EHD by 11-22-10. Class 2 Repeat Violation <br /> #27 One of the three five gallon buckets of used oil in the shop was left open. All hazardous waste <br /> containers shall be closed at all times except when adding or removing waste. Brian Camper corrected <br /> this during the inspection by closing the open bucket of used oil. Continue to ensure all hazardous waste <br /> containers are closed when not adding or removing waste. Class 2 Repeat Violation <br /> #52 An emergency coordinator and modified contingency plan information is lacking. There must be at <br /> least one emergency coordinator on site or on call to coordinate emergency response measures, and <br /> the following information must be posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm, and the <br /> phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br /> emergency coordinator and post the required information by a phone. A form is provided that can be <br /> used for this purpose. Submit a copy of the information to the EHD by 11-22-10. Minor-Notice to Comply <br /> #60 & 63 Copies of hazardous waste disposal records for 2007, 2008, 2009, and 2010 were not found <br /> on site.
