Algarmi I' <br /> 5650 Market Street <br /> Oakland, California 94608 <br /> Subject: Former Shell Service Station <br /> 1313 East Charter Way <br /> Stockton, California 95205 <br /> The San Joaquin County Environmental Health Department (EHD) has received and <br /> reviewed Well Destruction Work Plan (work plan), dated 29 April 2011, submitted by j <br /> Conestoga-Rovers & Associates (CRA) for the above-referenced site on your behalf. The j <br /> work plan was submitted in response to the EHD letter dated 06 April 2011 directing the <br /> submittal of a work proposal that would detail thQ methods and materials to be used to <br /> destroy the monitoring wells at your site. <br /> In the work plan, CRA proposes to drill out monitoring wells MWA and MW-6 to their <br /> original depths using ten-inch augers and fill the boreholes with grout. CRA proposes to <br /> destroy the remaining T on- and off-site monitorin wells by removing the vault box, then <br /> excavating a hole one foot larger in diameter than he original borehole to a depth of 3 feet <br /> below surface grade (bsg). The well casing will hen be cut off 2.5 feet bsg (6 inches <br /> above the bottom of the excavation), and then 9 e well will be filled with grout and the <br /> grout will be allowed to spill over the top of the casing and fill the hole to a thickness of <br /> one foot (6 inches above the well casing). CRA does not mention that the grout to be <br /> used to fill the casing will be applied under pressure prior to allowing the grout to spill over <br /> and fill the surrounding hole. The EHD directs that the grout be applied under liquid <br /> pressure of 25 pounds per square inch (psi) for a minimum of 10 minutes. With this <br /> condition, the EHD approves the work plan. <br /> Please provide a copy of the access agreemeni signed by the owner of the adjacent <br /> parcel where three wells are located, a completed Master File Record form, a Well and <br /> Boring Permit Application form for each parcel where wells are located, and a <br /> permit/inspection fee of $1830.00. Notify the EH D of pending fieldwork at least a week x <br /> prior to the well destructions. <br /> Y. <br /> .I <br />
