Time In: 11:15 am <br /> Time Out: 12:26 pm <br /> Food Program Complaint Inspection Report <br /> Name of Facility: CHICAGO'S PIZZA WITH A TWIST Date: 11/02/2018 <br /> Address: 100 W TURNER RD, LODI 95240 <br /> Owner/Operator: MATHFALLU, SALBINDAR Telephone: (209)365-1142 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00047694 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES THE CHICKEN ON THE PIZZA THEY ORDERED WAS COLD, COOKS IN THE KITCHEN <br /> WERE NOT WEARING HAIR NETS,AND THE WORKERS WERE HANDLING CASH AND THEN FOOD WITHOUT <br /> WASHING THEIR HANDS. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> As per person in charge, she is not aware of the complaint. <br /> I asked how they are preparing pizza.They pull toppings, pre cooked from the cooler(41 F), add toppings on dough and bake <br /> it. I asked them to prepare chicken pizza and I checked the temp of the chicken which was 184F. <br /> As per person in charge they only prepare pizza by order even in happy hour(Buffet 3 hours duration). <br /> I observed a customer came in to pick up his pre ordered pizza that was delivered to him in a box.
