<br />OBSERVATIONS: About 20 flies in kitchen. Close door, seal cracks between wooden panels at counter, and provide more
<br />pest control (i.e. replace or repair fly zapper over the prep sink and keep it on, or other forms of non-toxic pest control)
<br />2 desiccated dead mice in trap station #2. Dispose immediately. Observed manager placed trap in a bag and disposed to
<br />dumpster outside.
<br />20 to 30 mice droppings under wooden stool at storage shelf in dry storage. Inactivate then dispose droppings; clean facility
<br />to remove any attraction and provide more pest control. This is a repeat violation.

<br />OBSERVATIONS: Lettuce not covered, and flies hovering around 1D prep table. Cover and take action to protect food
<br />from contamination.

<br />OBSERVATIONS: Sprayer at 3-comp sink continues to spray/leak after handle is turned off. Repair. This is a repeat
<br />violation.
<br />Faucet of prep sink is cracked, which is covered with a plastic cup secured with a piece of duct tape. Remove tape to faucet
<br />to provide cleanable and non-absorbent surface, and repair crack / leak in 1 week.
<br />Cracked ledges and surface on prep table at the prep sink in kitchen, exposing particle wood surface underneath. Repair or
<br />replace to provide only cleanable and non-absorbent surface in 1 week.
<br />Hood filters greasy. Clean in 1 week.
<br />Very dusty grill on floor fan. Clean grill before using in kitchen in 1 day.
