In response to Violation #2, Inspection Item#6—HSC25505(a)(1)Foiled to complete and/or <br /> submit a site map with all required content. <br /> Inspection Remarks: <br /> "The map does not show access and exit points(street access to the facility). A site map shall <br /> contain a north orientation,loading areas,internal roads,adjacent streets,storm and sewer <br /> drains,access and exit points,emergency shutoffs,evacuation staging areas, hazardous material <br /> handling and storage areas,and emergency response equipment. If a site map element is not <br /> applicable for your facility then list it on the map and label as"NA". Immediately log into the <br /> California Environmental Reporting System(CERS)at http://cers.ca.gov/ upload the correct or <br /> updated information,and submit to the EHD for approval. <br /> This is a minor violation." <br /> Resolution: Evacuation Map was updated to include the location of the street access to the facility <br /> parking lot and all access/exit points into/out of the facility. Updated map was submitted in CERS on <br /> 4/8/2019. See Attachment 1 and Attachment 3. <br /> In response to Violation #3, Inspection Item#7—HSC 25505(a)(3)Failed to establish and/or <br /> submit emergency response procedures for a release or threatened release. <br /> Inspection Remarks: <br /> "The emergency response procedures were lacking. The phone numberforthe Local Unified <br /> Program Agency,209-468-3420 has not been identified. A business plan shall include the following <br /> emergency response procedures for a release or threatened release of hazardous materials, <br /> including, but not limited to,the following: <br /> Immediate notification of local emergency personnel and the Environmental Health <br /> Department(EHD) <br /> Procedures for the mitigation of a release or threatened release to minimize any potential <br /> harm or damage to persons, property,orthe environment <br /> Evacuation plans and procedures, including immediate notice,for the site <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.ca.goy/, <br /> upload the correct or updated information,and submit to the EHD for approval. <br /> This is a Class II violation." <br /> Resolution: Emergency Response Action plan has been updated to include the phone number of the <br /> Local Unified Program Agency. California Emergency Response/Contingency Plan has been completed. <br /> Both documents have been loaded and submitted to CERS on 4/8/2019. Please see Attachment 1, <br /> Attachment 4,and Attachment S. <br /> 2 <br />
