In response to Violation #4, Inspection Item#8—CCR2658,2659 Failed to submit a training <br /> program or program is not reasonable or adequate forfaciliry. <br /> Inspection Remarks: <br /> 'The training program lacks the frequency of training(initial and annual refresher training), <br /> safety procedures in the event of release or threatened release of a hazardous material, <br /> familiarity with the emergency response plans and procedures, immediate notification contacts <br /> to the appropriate local emergency repose personnel and the Environmental Health <br /> Department,procedures for the mitigation of a release or threatened release to minimize any <br /> potential harm or damage to persons,property,or the environment. Training programs for all <br /> employees and annual training, including refresher courses,shall include safety procedures for <br /> the event of a release or threatened release of a hazardous material, including,but not limited <br /> to,familiarity with the facility's emergency response plans and procedures. These training <br /> programs may take into consideration of each employee. Immediately prepare or update the <br /> facility's training program,log into the California Environmental Reporting System(CERS)at <br /> http://cem.ca.gov/ upload the correct or updated information,and submit to the EHD for <br /> approval. <br /> This is a Class II violation." <br /> Resolution:Training program has been submitted in CERS on 4/8/2019, Program includes initial and <br /> refresher training for all employees to be completed within myKPAonline, KPA's online training <br /> platform. Please see Attachment 1 and Attachment 6. <br /> In response to Violation # 5, Inspection Item#9—HSC 25508(a)(1)Failed to annually review <br /> and certify that the business plan is complete,accurate, and up to date. <br /> Inspection Remarks: <br /> "The business plan information has not been reviewed and resubmitted to the California <br /> Environmental Reporting System(CERS)annually by January 15. The hazardous materials <br /> inventory shall be submitted by January 15 of each calendar year and may be submitted <br /> beginning November 1 of the previous year. Immediately log into the CERS at <br /> htti)://cers.calepa.ca.Rov/,enter the correct or updated information,and submit to the EHD for <br /> approval. <br /> This is a Class II violation." <br /> Resolution:All submittal elements were submitted on 1/29/2019 and accepted on 3/14/2019. On <br /> 4/8/2019 all submittal elements were updated,and resubmitted, in accordance with the Hazardous <br /> Material Program Inspection Report dated 3/15/2019.
