In response to Violation #4, Inspection Item#8—CCR2658,2659 Failed to submit a training program or program is not reasonable or adequate forfaciliry. Inspection Remarks: 'The training program lacks the frequency of training(initial and annual refresher training), safety procedures in the event of release or threatened release of a hazardous material, familiarity with the emergency response plans and procedures, immediate notification contacts to the appropriate local emergency repose personnel and the Environmental Health Department,procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons,property,or the environment. Training programs for all employees and annual training, including refresher courses,shall include safety procedures for the event of a release or threatened release of a hazardous material, including,but not limited to,familiarity with the facility's emergency response plans and procedures. These training programs may take into consideration of each employee. Immediately prepare or update the facility's training program,log into the California Environmental Reporting System(CERS)at http://cem.ca.gov/ upload the correct or updated information,and submit to the EHD for approval. This is a Class II violation." Resolution:Training program has been submitted in CERS on 4/8/2019, Program includes initial and refresher training for all employees to be completed within myKPAonline, KPA's online training platform. Please see Attachment 1 and Attachment 6. In response to Violation # 5, Inspection Item#9—HSC 25508(a)(1)Failed to annually review and certify that the business plan is complete,accurate, and up to date. Inspection Remarks: "The business plan information has not been reviewed and resubmitted to the California Environmental Reporting System(CERS)annually by January 15. The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at htti)://cers.calepa.ca.Rov/,enter the correct or updated information,and submit to the EHD for approval. This is a Class II violation." Resolution:All submittal elements were submitted on 1/29/2019 and accepted on 3/14/2019. On 4/8/2019 all submittal elements were updated,and resubmitted, in accordance with the Hazardous Material Program Inspection Report dated 3/15/2019. See Attachment 1 and Attachment 7. 3
