Submit 3 COMT)Iete sets of drawings illustrating the locations of all existing and proposed <br /> structures, including existing UST(s), in non-crasabie print to PUS-EFID. Upon approval, <br /> one set will be retained by PHS-ERD and !he other two will be returned, <br /> b. Include equipment and materW specification, sheets and manufacturer's brochures describing <br /> Field testing procedur-es, operating and mouitorin-5, systems and listing numbers or other <br /> evidence of a nationally recognized testing organization approval. All proposed leak <br /> detection methods and equipment must meet the requirements specified in 40 CFR 280.40 <br /> and have been reviewed by the State Water Resources Control Board Staff. <br /> c All ince.-sti6al monitors mast g4rform according to manufacturer's standards, and must be <br /> tested annually in accordance with section 2630 (d), California Code of RegulariorAs (CCR), <br /> Title 23 Waters, Division 3. Chapter 16, <br /> d. Include a detailed monitoring and response plan in accordance with secdon 2632 (d) (1) & <br /> (2), CCR, Title 23 Waters, Division 3, Chapter 16- <br /> A The plans should include a.cross sectional diagr= of tiv rank and piping system including <br /> secondary containment. overflow protection equipment, and extension of all pipes and/or <br /> ports from bellow grade to findsh grade. <br /> f. Submit a completed "PHS-EHD Service Request" form and the State Watn. Resources <br /> Control Board "Facilit)r Permit (A)" and "Tank Permit (B)" apPiicadOn farms. <br /> SITEMMGATION: <br /> a. In the event contamination is observed, -onfumed or suspected as aL result of a leaking UST <br /> system it is the resporsibifiry of the owner or operator to submit a workplan to PHS-Eli't <br /> Site Mitigation Unit prior to initiating any assessment or rernediation activities. To minimue <br /> delays, a Corrective Action Plan may be submitted concurrent with tank closure or <br /> installation pians.
