PHASE II HEALTH AND SAFETY PLAN
History: Summarize below. In addition to history, include complaints from public, previous
agency actions,known exposures or injuries,etc.
The subject property is a 99-unit apartment building constructed in 1984 and 1985;it currently consists of 17
one and two-story residential buildings,a rental office building,a swimming pool,a hot tub/spa,waterscaping
and ponds, and landscaping. Between 1947 and 1979,portions of the subject property were occupied by the
Lincoln Village Water Pollution Control Plant which contained five 4-to 6-foot deep,unlined oxidation ponds;
these were used for the treatment of domestic sewage only. Aerial photographs indicate that most of the Country
Hollow Apartment property was constructed over areas formerly occupied by the Plant's access road, office and
other above grade structures. However,a small part of the property was placed over portions of the treatment
plant's oxidation ponds. Therefore, subsurface soils may contain metals, total petroleum hydrocarbons, and
volatile organic compounds.
Waste Types:
[ ] Liquid [ ] Sludge [ ] Unknown
x] Solid [ ] Gas [ ] Other: (Specify)
[ ] Gasoline
Waste Characteristics: Check as many as applicable.
[ ] Corrosive [ x] Inert ( ] Reactive
[ ] Toxic [ ] Volatile [ ] Radioactive
[ ] Flammable [ x] Unknown [ ] Other: (Specify)
Yr
Hazards of Concern:
[ ] Heat Stress-Attach [ x J Mechanical(drill rig)
Guidelines(heatstrs.gid) [ ] Biological
[ ] Cold Stress-Attach [ ] Inorganic Chemicals
Guidelines(cold.gid) [ x] Organic Chemicals
Explosion/Flammable [ x J Noise
[ ] Oxygen Deficient [ ] Radiological
[ ] Carbon Monoxide [ ] Security
[ ] Excavation,shoring,trenching
[ ] Other: (Specify)
1..1
Principal Disposal Methods and Practices: Summarize below.
