The <br /> 323 sensor and the external AN alarm were installed and tested on May 22, 2009 during the <br /> monitoring system certification. The relay module is part of the external overfill prevention AN <br /> alarm and was installed on June 2, 2009. <br /> The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Garrett Backus to arrange the following inspections(a 48 hour notice is <br /> required): <br /> a. Verify that the overfill prevention alarm is set at 90%of the tank capacity for all <br /> of the tanks. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid,current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC),and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested,repaired, or serviced,and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 5. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site,at all times while work is being performed, <br /> who possesses all of the following: <br /> Page 1 of 2 <br />
