A penetration fitting was replaced in UDC 5/6 under permit number SR0074967 in August 2016 and the UST Certification of Installation/Modification has not been submitted. This certification is required to be submitted within 30 days of completion of work to the California Environmental Reporting System(CERS). Immediately log into CERS, complete the required information, and submit for review by the EHD. This is a minor violation. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to visually inspect the 87 master fill sump for leaks, water, or debris during the May 2017 monthly inspection. The work order for the L6 (87 master fill sump)fuel alarm indicated that the liquid in the sump was removed by the facility and the technician did not open the sumps. During the monthly inspection, the designated operator shall visually inspect all spill containers and UDC sumps, and any tank top sump that had an alarm for which there is no record of a service visit and inspection by a qualified service technician. Ensure that designated operators performing monthly inspections at this facility are inspecting all of the required areas and noting it on the on the reports. This is a minor violation. 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. Fuel drips were noted on steel product piping under dispensers 1/2 and 7/8, indicating a leak in the primary piping. Moisture was observed on the floor of the UDC under the dripping piping in UDC 7/8. All primary containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component under permit and inspection of the EHD. This is a minor violation. -- _.......... _- 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). Liquid was observed in UDC 1/2. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. The service technician removed approximately 1/2 gallon of liquid from UDC 1/2. Ensure that all sumps and annular spaces are maintained free of liquid. This was corrected on site. This is a repeat violation, Class ll. Overall Inspection Comments: i This report has been amended on 1/4/18 to add violation #120. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by February 2, 2018.
