The following is an itemized list of hazardous materials violations that have not been <br /> addressed for GILTON RESOURCE RECOVERY INC as of May29, 2019. <br /> Open violations from March 29, 2019 inspection <br /> Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> One 55 gallon drum of waste oil was found on site that has not been reported. Any material that meets or exceeds <br /> the reportable quantity shall be reported in the facility's business plan. Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material <br /> inventory, and submit to the EHD for approval. <br /> The Hazardous Materials Inventory forms for the bioside HS 15%, carbachol chloride, acetylene, sodium hydroxide <br /> 50%, hydrogen peroxide, liquefied petroleum gas (Ipg), and motor oil are not complete. <br /> 1. The chemical location, days on site, average daily amount, and federal hazard categories have not been <br /> completed for the bioside HS 15%. <br /> 2. The chemical location, days on site, size of the largest container, average daily amount, storage pressure, <br /> storage temperature, and federal hazard categories have not been completed for the carbachol chloride. <br /> 3. The chemical location, days on site, size of the largest container, average daily amount, storage pressure, and <br /> the storage temperature have not been completed for the acetylene. <br /> 4. The days on site, average daily amount, storage pressure, storage temperature, and federal hazard categories <br /> have not been completed for the sodium hydroxide. <br /> 5. The days on site and the average daily amount have not been completed for the hydrogen peroxide. <br /> 6. The storage pressure has been wrongly identified as ambient vs. above ambient for the Ipg. <br /> 7. The storage pressure and storage temperature have not been competed for the motor oil. <br /> This information must be current and submitted as part of the business plan. Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.caleypa.ca.gov/, enter the correct or updated information, <br /> and submit to the EHD for approval. <br /> Violation #6 - Failed to complete and/or submit a site map with all required content. <br /> The map does not show access and exit points (structure), emergency response equipment(fire extinguishers), and <br /> a legend to show all of the codes/symbols used on the map. A site map shall contain a north orientation, loading <br /> areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, <br /> evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If <br /> a site map element is not applicable for your facility then list it on the map and label as"NA". Immediately log into <br /> the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated <br /> information, and submit to the EHD for approval. <br /> Page 1 of 1 <br />
