DTSC has on record your submittal form <br /> dated February 6, 2008 and an affidavit signed by John Sherburne on June 27, 2008 <br /> attesting that the property meets the requirements of the Taxpayer Relief Act. The Site <br /> is on the State Water Resources Control Board Ge Tracker Database as Case <br /> #391161. The Geotracker website for this Site contains electronic copies of quarterly <br /> groundwater monitoring reports for sampling conducted beginning in 2004. <br /> This property is located at 2448 Kettleman Lane West (BEACON 67080) in Lodi, San <br /> Joaquin County, California 95240. The Public Heal h Services of San Joaquin County <br /> Environmental Health Division (SJCEHD) is overseeing the investigation and cleanup of <br /> this Site. Results of soil and groundwater investiga ions conducted between 1985 and <br /> 1996 detected elevated concentrations of petroleum hydrocarbons, including total <br /> petroleum hydrocarbons as gasoline (TPHg), benzene, ethylbenzene, toluene and <br /> xylene (BTEX). Remedial actions have included soil excavation to remove petroleum- <br /> impacted soil,from the UST pit and product line locE tions, groundwater pumping and <br /> treating from January 1986 to late 1,988 and soil vapor extraction and treatment from <br /> December 1989 to April 1994. Following completio i of remedial activities, the Site was <br /> granted case closure by SJCEHD in August 1997. baseline assessment was <br /> conducted in 2003 that found MTBE in soil and groL ndwater underlying the Site. <br /> Subsequent groundwater monitoring through May 2 07 found MTBE in groundwater <br /> underlying the Site. Therefore, this property meets the terms for a "qualified <br /> contaminated site," as set forth by the Taxpayer Re ief Act.
