Since you requested
CVRWQCB be the lead oversight agency and they have complied with your request, this case must be
removed from the Local Oversight Program.
San Joaquin County Ordinance Code requires well permits be obtained from EHD whenever any type of
well/boring are to be constructed including those for environmental investigation and that inspections of the
work be performed. EHD must charge for the staff time spent inspecting and permitting these wells/borings.
The current hourly rate charged by EHD is$89. In order to process any future well/boring permit
applications needed to conduct work required by the CVRWCB, an account must be established with EHD.
Enclosed is a Master File Record (MFR)for you to complete and return to EHD with a payment of$267
(minimum 3 hours). This payment will be credited to your account code for this site and is required before
the next well/boring permit can be issued. After the first three hours of work, staff time on your site will be
multiplied by the hourly rate, and you will be billed for the calculated amount.
The CVRWQCB will continue to comment, direct, and approve investigative and remedial proposals and
EHD will continue to process, issue, and conduct grout inspections as required by San Joaquin County
Ordinance Code.
Please contact Michael Infurna at(209)468-3454 if you have any questions. Mr. Infuma can also be
reached by email at minfurna(&sicehd.com.
Donna Heran, REHS, Director
Environmental Health Division
A4 ZZtJr.,S for REHS ono, REHS
Site Mitigation Unit IV Supervisor
MI/
enclosure(to addressee only)
c: CVRWQCB-Marcus Pierce,Sacramento.
c: UpGradient Consulting—Vic Cherven, PO Box 8002 Stockton 95208
