ONE OF THE MEALS, <br /> SUPER STAR COMBO HAD A COOKED COCKROACH IN THE HAMBURGER ON A PICKLE. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. No cockroaches observed in restaurant. Looked through several places where <br /> pickles are stored and did not find any foreign item or bugs inside.
