Box 388 • Stockton, CA 95201-0388 q�iFoaN <br /> 209/468-3420 <br /> June 15, 1995 (C(DPY <br /> Stockton Service Station Equipment Company Inc. <br /> Attn: Mark Henderson <br /> Union Street <br /> Stockton, CA 95201 <br /> RE: Office Hearing held June 5, 1995 at 1:30 pm regarding tampering with a required inspection <br /> and refusal to allow an inspection at the Flag City Chevron. <br /> The above office hearing was held in order to avoid any further legal action in the above matter and get <br /> the above tank system into service the following agreements were made: <br /> 1. All four sumps at the site will be 24 hr. lake tested and witnessed by PHS-EHD using evidence <br /> tape to prevent any possibility of tampering. <br /> 2. The required tank integrity test at the above site will witnessed by PHS-EHD. <br /> 3. As per Mr. Henderson, Mr. Jim Cornell will not be at this site or any other site when PHS-EHD <br /> personnel are making inspections. <br /> 4. The providing of proof that Mr. Comell has been trained and certified by Centron pipe company <br /> to install their pipe products. (Section 2635 (d)(1) CCR Title 23) <br /> Note that this hearing was held to make agreements in order to preclude the filing of a legal case <br /> against Stockton Service Station Equipment Inc. Failure to meet any of the above requirements, <br /> agreements or the refusal to allow an inspection will result in further legal action. No further notice or <br /> hearing will be provided by PHS-EHD prior to filing of any legal case against your company. <br /> If you have any questions regarding this matter, please call me at (209) 468-3446. <br /> Ernest M.
