PUBLIC HL*ALTH SERVICtS
SAN JOAQUIN COUNTY
ENVIRONMENTAL HEALTH DIVISION
Karen Furst, M.D., M.P.H., Health Officer
304 East Weber Avenue, Third Floor • Stockton, CA 95202
209/468-3420
October 6, 2000
Mr. Richard Bokides
Bokides Family Ltd. Part.
6421 Capitol Avenue
Lodi, CA 95242
RE: Flag City Chevron, 6421 Capital Ave
AXE,
IL
1 1' 1 l
FILE COPY
As per our letter of August 17, 2000 a tracer test was requested as part of methods needed to determine
where a possible ongoing release was located in your piping system. It is imperative that a test be made of
this system in order to determine the cause of the new release described by the RWQCB. Note that only
tests listed in LG 113 by the SWRCB will be an acceptable test. I have enclosed a copy of a page from LG
113, which describes the only approved test(s) available, which can test for new releases exterior to the
product piping lines. If you are unable or you do not want to do the tracer test, you may uncover and test
soil around exposed lines to determine where a failure has occurred. A permit will be required for any
sampling or exposing of existing lines. Failure to comply with the Under Ground Storage Tank law can
result in loss of your operating permit and the lost of funding from the SWRCB. You must meet the
"Reporting, Investigation, and Initial Response Requirements for Unauthorized Releases" CCR Title 23
Section 2652.
You must contract the EHD within five (5) days of this date of this letter, with your intentions in this
matter.
All work must be completed in 15 days of the date of this letter in order to prevent any further legal action.
If you have any further questions, please contact me at (209) 468-3446.
Sincerely
Doug Wilson, Supervising REHS
Environmental Health Division
cc: Mr. Mike Infurna, Senior REHS
Mr. Micheal Kith, Senior REHS
Mr. Timothy J.
