The following is an itemized list of underground storage tank violations that have not <br />been addressed for FLAG CITY CHEVRON as of December 08, 2017. <br />Open violations from September 26, 2017 inspection <br />Violation #201 - Failed to maintain alarm logs and/or records of follow up actions. <br />Maintenance and monitoring records for the last three years were not found on site. The last three years of <br />monitoring system certifications, and secondary containment testing were not on site. Facility has no alarm logs, <br />and the designated operator monthly inspection reports for October 2016 through May 2017 were not on site. These <br />records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all <br />monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual <br />observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other <br />monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br />reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on <br />site and submit copies to the EHD. <br />Violation #210 - Designated operator monthly inspection reports not maintained on site for at least 12 <br />months. <br />The October 2016 through May 2017 designated operator monthly inspection reports were not found on site. <br />Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate <br />and ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br />maintained on site. Submit copies to the EHD. <br />Violation #306 - Monitoring equipment is not installed or maintained to detect a leak at the earliest <br />opportunity. <br />In the 87 -octane (master) submersible turbine pump sump, the drain plug was left in place at the test boot. As such, <br />any potential release from the primary containment into the secondary containment in the 87 -octane (master) piping <br />does not flow into a collection sump. All secondary containment systems shall be sloped so that all releases will <br />flow to a collection sump located at the low point of the underground piping. Immediately contact a properly <br />licensed, trained, and certified contractor to ensure that any releases in the 87 -octane piping flows into the sump <br />with a sensor. Submit proof of correction to the EHD. <br />Violation #319 - No spill bucket, failed to contain five gallons, or failed to provide a means to keep it empty. <br />At the start of inspection there was approximately 1/2 inches of free standing liquid in the diesel spill container. <br />Some liquid was also observed in the 87 (master) spill container, 91 spill container, and 87 (slave) spill container. All <br />USTs shall be equipped with a spill container that has a minimum capacity of five gallons and a drain valve that <br />allows drainage of the collected spill into the primary container or provide a means to keep the container empty. All <br />liquids were removed by the service technician from these spill containers at the time of inspection. Maintain all spill <br />containers free of liquid at all times. This was corrected on site. <br />The 87 -octane (master) spill container failed when tested. All spill containers shall have a minimum capacity of five <br />gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately contact a <br />properly licensed, trained, and certified contractor to repair or replace the spill container under permit (as required) <br />and inspection of the EHD. <br />Page 1 of 1 <br />
