The following is an itemized list of underground storage tank violations that have not
been addressed for FLAG CITY CHEVRON as of December 08, 2017.
Open violations from September 26, 2017 inspection
Violation #201 - Failed to maintain alarm logs and/or records of follow up actions.
Maintenance and monitoring records for the last three years were not found on site. The last three years of
monitoring system certifications, and secondary containment testing were not on site. Facility has no alarm logs,
and the designated operator monthly inspection reports for October 2016 through May 2017 were not on site. These
records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all
monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual
observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other
monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and
reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on
site and submit copies to the EHD.
Violation #210 - Designated operator monthly inspection reports not maintained on site for at least 12
months.
The October 2016 through May 2017 designated operator monthly inspection reports were not found on site.
Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate
and ensure that copies of the previous twelve months of designated operator monthly inspection reports are
maintained on site. Submit copies to the EHD.
Violation #306 - Monitoring equipment is not installed or maintained to detect a leak at the earliest
opportunity.
In the 87 -octane (master) submersible turbine pump sump, the drain plug was left in place at the test boot. As such,
any potential release from the primary containment into the secondary containment in the 87 -octane (master) piping
does not flow into a collection sump. All secondary containment systems shall be sloped so that all releases will
flow to a collection sump located at the low point of the underground piping. Immediately contact a properly
licensed, trained, and certified contractor to ensure that any releases in the 87 -octane piping flows into the sump
with a sensor. Submit proof of correction to the EHD.
Violation #319 - No spill bucket, failed to contain five gallons, or failed to provide a means to keep it empty.
At the start of inspection there was approximately 1/2 inches of free standing liquid in the diesel spill container.
Some liquid was also observed in the 87 (master) spill container, 91 spill container, and 87 (slave) spill container. All
USTs shall be equipped with a spill container that has a minimum capacity of five gallons and a drain valve that
allows drainage of the collected spill into the primary container or provide a means to keep the container empty. All
liquids were removed by the service technician from these spill containers at the time of inspection. Maintain all spill
containers free of liquid at all times. This was corrected on site.
The 87 -octane (master) spill container failed when tested. All spill containers shall have a minimum capacity of five
gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately contact a
properly licensed, trained, and certified contractor to repair or replace the spill container under permit (as required)
and inspection of the EHD.
