Any release from the primary containment that does not escape the secondary <br /> containment and was cleaned up within eight hours of being detected shall be recorded with the following information: <br /> 1. Operator's name and phone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean the release <br /> 4. Method and location of disposal of the released substances <br /> 5. Description of actions taken to repair and prevent future releases <br /> 6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary <br /> containment <br /> Immediately record this release with the information required, and maintain it on site with the facility's monitoring <br /> records. Submit a copy of the recordable release statement to the EHD. <br /> This is a Class II violation. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The vent box sensor failed to go into alarm during testing. Technician was able to manually adjust on site and the <br /> sensor began to work. <br /> All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the <br /> dispenser when it detects a leak. <br /> Corrected on site <br /> This is a Class II violation. <br /> 319 CCR 2635(b) No spill bucket, failed to contain five gallons, or failed to provide a means to keep it empty. <br /> At time of inspection the diesel spill container had approximately 11/2 inches-2 inches of freestanding liquid in it. The <br /> diesel spill container failed to have a minimum capacity of five gallons. All spill containers shall have a minimum <br /> capacity of five gallons. <br /> Spill containers must be kept free of debris and liquid and be able to contain a minimum of 5 gallons. Technician on <br /> site removed the liquid. <br /> Corrected on site. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by October 26, 2016. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> Page 4 of 5 <br />
