The container for drained used oil filters was only labeled as used oil filters. Used oil filters must be <br /> stored in a closed, rainproof container, labeled as "drained used oil filters," and marked with an <br /> accumulation start date. Immediately add the word drained to the existing label and ensure that all used <br /> oil filters are managed in this way. <br /> 703. The emergency coordinator information is lacking. There must be at least one emergency <br /> coordinator on site or on call to coordinate emergency response measures, and the following information <br /> must be posted by a phone: the name and phone number of the emergency coordinator, location of fire <br /> extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire <br /> department, unless the facility has a direct alarm. Immediately appoint an emergency coordinator and <br /> post the required information by a phone. A form is provided that can be used for this purpose. Submit <br /> proof of correction to the EHD by 2-13-14. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by 2-13-14- <br /> The complaint inspection also included a site visit to the SJC Mosquito & Vector Control White Slough <br /> facility at 12751 N. Thornton Rd. Lodi, CA 95242. <br /> Gary McIver Jr., Senior Safety Engineer from Cal/OSHA was also present during the inspection and <br /> stated that the only issue of concern at this time is that SJC Mosquito & Vector Control maintain a <br /> material data sheet for Paracide - F. Gary McIver stated that there are no other Cal/OSHA concerns for <br /> this facility at this time. <br /> Robert Lopez, Hazardous Material Specialist II from SJCEHD was also present during the inspection. <br /> Refer to the hazardous materials inspection report. <br /> John Fritz, Assistant Manager for SJC Mosquito & Vector Control District confirmed that Formalin and <br /> Paracide - F are no longer used by SJC Mosquito & Vector Control District since 2009. <br /> During the inspection, employee training records for hazard waste management, the emergency <br /> contingency plan, and material data sheets were reviewed. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO EINSPECTION AT ANY T ME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By' Title: <br /> Aris Cacapit (209) 468-3453 <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
