SUMMARY OF VIOLATIONS CLASS I, CLASS II,or MINOR-Notice to Comply) This is a report of the Routine hazardous waste inspection conducted at the referenced facility by San Joaquin County Environmental Health Department (EHD). The following are Class II (Repeat) Violations: 108, 122/123/124, 125, 127, 128, 605/701 and the following are Minor Violations - Notice to Comply: 606, 607, 610, 611, 803/806/807/808/809, 812. 108 (Repeat). One black 5-gallon bucket of unknown waste with a completely faded hazardous waste label, and multiple containers of paint waste at the court yard (outside the work shop) require hazardous waste determination. Generator shall make hazardous waste determination of the waste and manage the waste accordingly. Immediately make a hazardous waste determination of the above wastes and manage each accordingly, or dispose of the waste as hazardous waste in accordance to California Code of Regulations (CCR) Title 22. Submit a statement and supporting documentation explaining how this waste was managed by 02/13/2014. 122, 123, & 124 (Repeat). Manifest number 007302564JJK was not found on site, and Manifest number 0 , 004384647FLE, and 004863800FLE were found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests for three years or until a copy signed off by the destination facility is received. The signed off copy shall be retained and made readily available for review for three years. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received continues on page 5 ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
