Continue to ensure that all UW is properly managed per Title 22 Hazardous Waste Regulations. <br /> Corrected on site. <br /> 809. The facility did not demonstrate when the four UW lamps on site became a UW. A UW handler <br /> shall accumulate UW for no longer than one year from the date the UW was generated, or was received <br /> from another UW handler. A UW handler shall be able to demonstrate the length of time that the UW has <br /> been accumulated from the date it became a waste or was received. The UW handler may make this <br /> demonstration by: (1) Placing the UW in a container and marking or labeling the container with the <br /> earliest date that any UW in the container became a waste or was received. (2) Marking or labeling the <br /> individual item of UW with the date it became a waste or was received. (3) Maintaining an inventory <br /> system on site that identifies the date the UW being accumulated became a waste or was received. (4) <br /> Maintaining an inventory system on site that identifies the earliest date that any UW in a group of items <br /> of UW or group of containers of UW became a waste or was received; (5) Placing the UW in a specific <br /> accumulation area and marking or labeling the area to identify the earliest date that any UW in the area <br /> became a waste or was received; or (6) Any other method which clearly demonstrates the length of time <br /> that the UW has been accumulated from the date it became a waste or was received. During the <br /> inspection John Anagnos marked the container of UW lamps with an accumulation start date. Continue <br /> to ensure that all UW is properly managed per Title 22 Hazardous Waste Regulations. Corrected on site. <br /> Note: John Anagnos requested what are the UW disposal record requirements. We discussed the <br /> following: A UW handler shall keep a record of each shipment of UW sent from the UW handler's facility <br /> to another facility. The record may take the form of a log, invoice, manifest, bill of lading or other <br /> shipping document. The record for each shipment of UW sent shall include the following information: <br /> (1) The name and address of the UW handler or destination facility to which the UW was sent; <br /> (2) The quantity [count or weight, consistent with, for example, section 66273.32, subsection (d)] of each <br /> type of UW sent (e.g., batteries, thermostats, lamps, electronic devices, CRTs, CRT glass); <br /> (3) The date of departure of the shipment of UW. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by 3-2-12. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Receiv y: � Title: <br /> Aris Cacapit (209) 468-3453 r- <br /> EHD 23-02-003 Rev 07/12111 ONTINUATION FORM <br />
