Main St. <br />Stockton, Ca 95202-3029 <br />Mr. Naidu, <br />The purpose of this letter is to address the Class II Notice of Violations that were found <br />during the inspection on May 20, 2011. There were two violations discovered and both of those <br />violations have now been corrected. The fust one, number 45 on the checklist was the facility's <br />lack of location and description of the emergency equipment. As you will see in the attached <br />paperwork, I have annotated the location and description and color coded them accordingly on <br />the attached maps. The second violation was a missing manifest. I contacted the vendor and <br />discovered the reason we did not have the manifest and had no knowledge of the manifest was <br />due to the fact that it was a correction manifest. The vendor could not locate a barrel after the <br />original pickup. When it was eventually located, it was not on the original manifest so they <br />generated one. If you review the manifest, you will notice in the generators signature block it <br />was not signed by any personnel of this facility. I requested from the vendor that should this <br />occur again, they are to ensure that we, as the generator, receive a copy for our records.
