S. - PHS-EHD <br /> December 14 , 1990 <br /> Wayne Mannor, Sumiden Wire, confirmed the following hazardous <br /> wastes were being recycled. <br /> 1 . 1, 322 , 637 lbs sulfuric acid. <br /> 2 . 121, 860 lbs zinc phosphate filter cake. <br /> 3 . 1, 800 gallons waste oil and solvent. <br /> March 12 , 1990 <br /> Report of violation sent to Sumiden and Department of Health <br /> Services. <br /> April 10, 1990 <br /> Re-inspection made at request of Wayne Mannor, Sumiden Wire <br /> concerning report of violation. <br /> November 2 , 1990 <br /> Doug Wilson, Supervising R.E.H. S. , informed Mr. Olsen that <br /> billing for hazardous waste generators fees may not be correct <br /> due to Sumiden Wire not properly manifesting zinc phosphate <br /> and sulfuric acid from the site for recycling. <br />
