General Instructions for Completing this Questionnaire <br /> Your careful completion of this questionnaire is very important. Please answer each question to the best of your <br /> ability by checking the appropriate box Q or by filling in the requested information. Responses you provide to this survey <br /> should concern the subject facility indicated on the front of the questionnaire. Please keep in mind that your participation <br /> in the survey will assist in evaluating the performance of California's Department of Toxic Substances Control in <br /> implementing the Hazardous Waste Source Reduction and Management Review Act of 1989. <br /> Applicability of the Hazardous Waste Source Reduction and Management Review Act(SB 14) <br /> to the Subject Facility <br /> With respect to applicability of California's Hazardous Waste Source Reduction and Management <br /> Review Act to the subject facility, the Act applies only to generators who,by site, routinely <br /> generate through ongoing processes and operations, more than 12,000 kilograms of hazardous <br /> waste in a reporting year(or more than 12 kilograms of extremely hazardous waste in a calendar <br /> year). The following hazardous wastes streams should not be included in calculating the volume <br /> (or comparable weight)of waste produced, and are not subject to the Act: <br /> • Automotive fluids <br /> • Lead acid batteries <br /> • Household hazardous waste streams and wastes from household collection events <br /> • Wastes separated at community landfills <br /> • ,"aste pesticides and pesticide containers collected by County agricultural commissioners i <br /> • t munitions and ordnance <br /> I <br /> • _.,.nmissioned utility poles. <br /> Also, the following hazardous wastes streams that are not routinely generated should not be j <br /> included in calculating the volume (or comparable weight) of waste produced,and are not subject to <br /> the Act: <br /> • Waste from site cleanup and mitigation activities, including remedial investigations <br /> • Samples and evidence from enforcement actions <br /> • Asbestos <br /> • PCBs <br /> • Formation fluids and solids from oil, gas, and geothermal exploration and field <br /> development <br /> • Demolition waste/major renovation <br /> • Waste generated from emergency response actions. <br /> Finally, medical waste is exempted from the Act, and a generator may petition the Department of <br /> Toxic Substances Control in writing to exempt other hazardous waste streams. <br /> I <br /> Thank you for your cooperation.
