The information on this monitoring <br /> program are conditions of the operating permit. The permit holder must notify San Joaquin County Public <br /> Health Services within 30 days of any changes to the monitoring procedures, unless require"MOVE[D) <br /> approval before making changes. Required by Sections 2632(k) and 2641(h) CCR. <br /> Facility Name: Tesoro#67080 JUL 1 7 2002 <br /> Facility Address: 2448 W Kettleman Ln., Lodi California <br /> ENVIRONMENT HEALTH <br /> 1. Describe the frequency of performing the monitoring: PERMIT/SERVICES <br /> Tank: Sensors provide continuous monitoring. Station Manager reports alarms to maintenance <br /> dispatch. <br /> Piping: Same as Above. <br /> Dispenser Containment: Bravo Boxes have float switch that trips impact valve. <br /> 2. What methods and equipment, identified by name and model, will be using for performing the <br /> monitoring: <br /> Tank: Gilbarco EMC electronic sensors provide continuous interstitial monitoring. <br /> Piping: Gilbarco EMC electronic sensors provide continuous interstitial monitoring. <br /> Dispenser Containment: Bravo Boxes monitored continuously. <br /> 3. Describe the location (s) where the monitoring will be performed (facility plot plan should be attached): <br /> Tank interstitial space. Turbine sumps for piping. <br /> 4. List the name (s) and title (s) of the people responsible for performing the monitoring and/or <br /> maintaining the equipment: <br /> Beth Portscheller, Station Manager and Cesar Bosano, Area Manager or Robert Hoover, <br /> Retail Environmental Administrator and Rose Pedregosa, Retail Environmental Coordinator <br /> at central office in Auburn, WA. <br /> 5. Reporting format for monitoring: <br /> Tank: Monitoring data is stored electronically on site. <br /> Piping: Same as Above. <br /> 6. Describe the preventative maintenance schedule for the monitoring equipment. Note: maintenance <br /> must be in accordance with the manufacturers' maintenance schedule but not less than every 12 <br /> months. <br /> Equipment will be tested and inspected following Veeder-Root's Periodic Maintenance <br /> Checklist every 12 months. <br /> 7. Describe the training necessary for the operation of UST System, including piping and monitoring <br /> equipment: <br /> Employees are trained on dispenser, POS System, Giibarco EMC System, overfill alarms <br /> and fuel delivery upon hire and refreshed every 12 months. <br />
