Kettleman Lane Lodi, CA 95240 Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I CLASS II or MINOR-Notice to Comply) <br /> Revised inspection report 6-2-11. <br /> On site to conduct a routine inspection and witness the annual monitoring system certification testing. <br /> 15. The 1-25-11 secondary containment test results indicate a failure at the 87, 89, 91 lines and turbine <br /> sumps. All secondary containment for the UST system must be tight. These failures have already been <br /> corrected and witnessed to pass under permit and inspection of the EHD. The six month secondary <br /> containment retest of the 89 and 91 turbine sumps are due by October 2011. The previous 89 tank has <br /> been converted to 91 and the previous 91 tank has been converted to diesel. Therefore, the six month <br /> secondary containment retest of the 91 and diesel turbine sumps are due by October 2011. Provide this <br /> office with a least a 48 hour notification prior to testing. <br /> 23c. The designated operator failed to attach the alarm history report for the L1 diesel turbine sump on <br /> the May 2011 designated operator monthly inspection report. The designated operator failed to attach <br /> the alarm history report for the L3 Plus turbine sump on the February 2011 designated operator monthly <br /> inspection report. The designated operator failed to attach the alarm history report for the L6 Plus <br /> annular space on the October 2010 designated operator monthly inspection report. During the monthly <br /> inspection, the designated operator shall review the alarm history for the previous month, check that <br /> each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> 999. CCR, section 2660 (k)(1)(m) The sleeve that holds the diesel turbine sump Veeder Root 208 sensor <br /> at the lowest point of the sump to detect a leak at the earliest opportunity has been drilled into the sump <br /> wall with a fastener. Pursuant to CCR, section 2660 (k)(1)(m) Local agencies shall not approve a repair <br /> or upgrade unless it can be demonstrated that the underground storage tank system is structurally <br /> sound and the method of repair or upgrade will prevent unauthorized releases due to structural failure or <br /> corrosion during the operating life of the underground storage tank system. The materials used in the <br /> repair or upgrading process shall be applied in accordance with nationally recognized engineering <br /> practices. Materials used in repairs and upgrades shall be compatible with the existing underground <br /> storage tank system materials and shall not be subject to deterioration due to contact with the hazardous <br /> substances being stored. Immediately have a properly licensed, trained, and certified contractor remove <br /> the screw and repair the diesel turbine sump wall under permit and inspection of the EHD. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by July 1, 2011. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: ReceWed By: (�I Title: <br /> II"'fi <br /> EHD 23-02-003 Rev OSi10H 0 CONTINUATION FORM <br />
