Lodi, CA 95242 SR0074954 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the application for a permit to install a Phil-Tite 85100-1 for the fill spill container and a Phil-Tite <br />85101-1 for the vapor spill container for the 87 and 91 products on the underground storage <br />tank (UST) system at the above noted address. The permit has been approved with the <br />following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Aris Veloso at (209) 468-3453 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Spill bucket test: Verify minimum 5 -gallon capacity and pass a one hour hydrostatic <br />test (or equivalent). Proper drainage must also be verified. <br />Please be aware that the facility cannot receive fuel in the 87 and 91 tanks prior to <br />the EHD inspection and approval. <br />4. Any individual performing the work of a service technician must meet the following <br />qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible <br />valid, current contractor's licenses issued by the Contractors State License Board <br />(may be issued to the installation company), or a Tank Tester's license issued to the <br />individual by the State Water Board, and <br />b.
