Lodi, CA 95242 SR0070748 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the application for a permit to install new (3 + 0) Gilbarco 700s fuel dispensers, install new <br />Gilbarco dispenser conversion frames, disconnect & reconnect all existing shear valve & <br />electrical for product and vapor, and upgrade Veeder Root Software from 1.02 to 1.05 version <br />with "Cold Start" on the underground storage tank (UST) system at the above noted address. <br />The permit has been approved with the following conditions.- <br />Contact <br />onditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Aris Veloso at (209) 468-3453 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Sensor functionality: Functionality of the monitoring system must be verified to <br />ensure that monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and the Health & Safety Code. A copy of the alarm <br />history shall be printed prior to performing the "cold start" of the Veeder Root system <br />and made available at the time of EHD inspection. <br />b. Visual inspection of the new dispenser and conversion frame to verify that no leaks <br />are present and that any leaks would be contained in the under dispenser <br />containment. <br />Please be aware that the facility cannot operate the UST system prior to the EHD <br />inspection and approval. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />
