From: Robert Lopez [EH] <br /> Sent: Friday, May 17, 2019 12:01 PM <br /> To: 'brown-anthonyl0@aramark.com' <brown-anthonyl0@aramark.com> <br /> Subject: FW: PR0519288; Aramark Uniform &Career Apparel, LLC Stockton Compliance Submittal Review. <br /> Mr. Brown, <br /> I have reviewed the compliance submittal and have found the following issues: <br /> • Violation#5, Inventory not complete. <br /> - Polyclear CD-580 4H (liquid), polyclear 390(solid), polyclear 390 (liquid), and hydraulic fluid have not <br /> been reported. <br /> - The hazardous component information for emax alkali, bleach, advacare, performance booster, injection <br /> sour, ecolab fabric relaxant,tex stat II, turbo crisp, polyclear 1267 has not been completed. <br /> - The storage pressure for Performance booster has not been completed. <br /> - The storage pressure and storage temperature for the injection sour and polyclear 1267 have not been <br /> completed. <br /> - The storage pressure for compressed oxygen, carbon dioxide, and acetylene is wrongly identified as <br /> "ambient'vs. "above ambient". <br /> - Dynalite (liquid)—the amounts are identified in "pounds" vs. "gallons". <br /> - Sodium sulfate (solid)—the amounts are identified in "gallons"vs. "pounds". <br /> - Polyclear 1267 -the amounts are identified in "pounds"vs. "gallons". <br /> - Used oil—the maximum daily amount is identified as 100 vs.the <br /> • Violation#6,the site map is not complete (blank map form attached). <br /> The site map does not address loading areas, internal roads, access and exit points (from the street), <br /> evacuation staging area,the hazardous materials storage and handling area of all of the hazardous <br /> materials required to be listed in the hazardous materials inventory. A site map shall contain a north <br /> orientation, loading areas, internal roads, adjacent streets,storm and sewer drains, access and exit <br /> points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, <br /> and emergency response equipment. If a site map element is not applicable for your facility then list it <br /> on the map and label as "NA". <br /> • Violation#8,this violation has not been addressed. <br /> • Violation#9 will remain open until violations 5, 6, and 7 have been completed. <br /> • Violation#12—will remain open until violation 5, 6, and 7 have been completed. <br /> Be advised, the violation comply by date has passed, consequently all time spent attempting to obtain compliance is <br /> required to be billed at$152 an hour. <br /> As of September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
