Happy Monday! And thank you for the clarification.
Just to ensure I understand, the 2017 violations have been cleared? We are currently working on the 2019
requirements? If so,the following is referring to 2019:
Before I submit the Employee Training Plan (Attached)to CERS, I just want to ensure the Employee Training Plan I
emailed to you on Friday does meet the necessary standards.
If so, I will delete the 4/18/19 submission of the incomplete Employee Training Plan that was submitted with The
Emergency Response/Contingency Plan; resubmit The Emergency Response/Contingency Plan; then upload the
Employee Training Plan in Section I.
As for the Employee Trainings, I have documentation for the Laboratory Employee Safety Training for Carbon Dioxide for
2018 and 2019 (Attached). The other annual Safety Trainings will take place shortly; when I have the documentation I
will forward them to you. (Is this the appropriate method? Or do I need to upload them to CERS?)
Thank you, again, for your help to resolve this issue.
--wlelissa Yago
209-468-9874
From: Robert Lopez [oes] <rlopez@slgov.org>
Sent: Friday, May 3, 2019 4:36 PM
To: Melissa Yago [PHS] <myago@sjcphs.org>
Subject: RE: Public Health CERS 10745503 Emp Training Documents
Mrs. Yago,
Emergency response plan and employee training plan are required to be uploaded to CERS. The Emergency
Response/Contingency Plan was uploaded CERS on 9/28/2017 and the violation was closed. The employee training plan
requirement was satisfied on 9/28/17 when the training element was submitted advising the Employee Training Plan
was provided elsewhere in CERS and is located in the Emergency Response and Training Plans (this is section I. Employee
Training Plan). Subsequently on 4/18/19,you uploaded an incomplete training plan to the Emergency
Response/Contingency Plan Section vs.the Employee Training Plan Section. This is why the Emergency
Response/Training Plan submittal on 4/18/19 was rejected. If you remove the incomplete training plan upload and
resubmit the to CERS,that will correct the CERS issue.
Please, provide copies of employee training records showing the required training has been completed to address the
past due violation for the failure to provide or document initial and/or refresher training to appropriate personnel.
Please, call me if you have questions or need clarification.
Respectfully,
Robert Lopez
Hazardous Materials Specialist II
San Joaquin County
Environmental Health Department
1868 E. Hazelton Ave.
Stockton, CA 95205
Work: (209) 953-6214
z
